Shot Records

All shots records need to be up-to-date by the first day of school!  No shots, No school!!!

Immunizations are a requirement for all students in the State of Texas. Students that do not have immunizations need to request an Exemption from Immunizations for reasons of Conscience. This form is can be requested through the Texas Department of Health and Human Services.
